PATIENT CARE AND COVERAGE
Our goal is to get your condition under control as quickly as possible, then help you maintain the progress you've made through infrequent follow-up visits. Of course, the timeframe and frequency of your visits will depend on your particular circumstances.
IMPORTANT: Because many factors can affect your condition over time, all patients are advised to come for a follow-up visit at least once a year, particularly if prescription renewals are needed.
Insurance
We accept all major credit cards, personal checks and cash. Your insurance company may reimburse you for my services as an out-of-network physician. You should find out ahead of time if this is the case. If your insurance company will be reimbursing you, we will provide you with a "superbill" with cpt codes and procedure codes for no-hassle submission to
your plan.
Fees
Initial Consultation: $650
Follow-up visits: $475 each
Brief follow-up visit: $175 (Discussion of results, lab test review
and treatment plan) up to 20 minutes
Payment
Mastercard, Visa, American Express and Discover are all accepted.
Personal checks are accepted. Please note that a small fee will be
assessed for any checks returned for insufficient funds.
